putting my two cents in because i have NPD AND low empathy. cognitive empathy exists. we can understand, rationally, what a person is going through, even if we don't full feel it. ("that's common sense, i fear" /ref)

also i have never (in my experience) met a narc who would publicly admit to their disinterest/lack of caring. the whole point is for you to trust me, why would i say something that would make me seem awful if it doesn't serve me?

(sharing my experience in this - i absolutely think that that racist loser who claimed to not care about the genocide was Not acting like someone with npd AT ALL because it was entirely image ruining and just disgusting behaviour altogether)

for me as an overt narcissist who’s diagnosed this largely depends on what image im trying to portray exactly to benefit me - if my goal is to make it known to others that i won’t take their shit & if i want them to view me in a certain way i Will openly express my lack of care in certain ways . i also have aspd so perhaps take my experience with a grain of salt but it definitely depends more on the desired image and outcome . i absolutely play more into a caring persona in most spaces so people do trust me , but as a narcissist with a lot of self awareness i can admit that i also do the opposite of that when i Know it’ll make me look better . i hope this makes sense ? i’m not discrediting you at all , just offering my perspective as a diagnosed narcissist & pwaspd and i didn’t want people without npd getting the wrong idea or base their view solely around what you said , but again i point to what i said at the start of this answer first and foremost 😭
