I like to imagine Sukuna's last finger just follows Inuyasha rules and incarnates a brand new Sukuna with no memories and Yuji has to basically raise a whole man, teaching him everything from speaking to reading, writing and basic survival skills, in the most clumsiest manner, but Sukuna picks up really fast, all while falling in love with Yuji, and Yuji's just like, "Wow, you're such a nice Sukuna! I knew you could do it!" Sukuna gets disappointed his grunts of affection went unnoticed. (Also Megumi is absolutely trying to kill Suku and get Yuji far away from him as possible- he's just so jealous)

Sukuna's problem is his mixed signals, he doesn't voice his affections (in normal words, not grunts etc) + his usual condescending way of talking and at the same time he's a clingy horny thing. yuuji just doesn't understand what sukuna wants from him.
in the end he'll have to use his words or simply grope and dryhump him
