Gojo being the first time to be intimate with him is 🤌 chef kiss (Sukuna's theme is all about not needing anyone else but himself to be satisfied and also goes into the sexual dimension). So Gojo, being the first to being the closest to "reach" him is a big thing in their dynamic. I don't think he is THAT clueless, probably understands the goal/basics but would have a lot of trouble just going for it. Gojo would probably taunt/tease him and that could either go wrong or just entice Sukuna to keep trying, but eventually Gojo takes the reigns and pretty much guides him through it.
Now it's one of Sukuna's favorite activities, but not the only one.
