It didn't take long for Medic to realized he liked Cheavy a lot, meanwhile Cheavy one day suddenly realizes his feelings with his head on his hands
That's very likely what happened. Medic latched on as initially a rebound for his previous Heavy but came to realize Cheavy is quite different and their wholesome moments are a spot of fresh air rather than his last more consistent and healthy relationship. Over time Cheavy no longer finds Medic's eccentricity or clinginess annoying, then he realizes he actually enjoys spending time with him and that he wants their relationship to be more than just transactional, then realizes he wishes their moments of wholesome were more frequent and tries to genuinely act nicer, initiate affection more. But the whole time he's internally beating himself up for letting himself fall for Medic of all people, not wanting to confront what that means about himself.
