As we don't collect any identifying information there isn't way for us to check if you've filled it out before, unfortunately. You could always try the following:

• Checking browser history to see if you've visited the form before.
• See if you've had a conversation with a friend where you mentioned you filled it out.

If those don't work, or you still feel unsure, I'd say go ahead and fill it out.
