I think Mikoto's case could still be the exact same if he was a woman, but if I put my own spin on it then here's how I would do it!:
Because men are statistically more likely to have job-related stress because of their income or how well they perform, I think F!09 (female 09, its easier for me to write LOL) could still have work related stress, but not because of how well she does, instead, it is bc of discrimination due to gender & harassment.
And instead of John being a "representation of what Mikoto wants to BE" (i think somewhere in neoplasm John says this) John could be a representation of someone F!09 wants to console in, since women are more likely to seek emotional support from others, they dont have as high expectations as men do in office jobs, but can be discriminated against because of their performance. She would likely still have parental issues, and her relationship with her sister could still be the same (instead of hard-working brother, it's a role-model sister dynamic! To show her sister that women can still do something like this & be successful) but her parents may look down on her decision & would have rather F!09 be a simple housewife, so she is unable to speak to them about her issues to not make their judgements become true. Which is the reason John would be formed
