Do you think Shido is privileged by the fandom?? He literally purposely failed multiple surgeries and harvested organs. But he was voted innocent in both trials. It shouldn’t matter who the organs were for, his body count might even be equal to Mikoto/John’s

I don't think so. Shidou is probably one of the most disliked prisoners, and the only reason he received two innocent votes is because most people that voted that way were using him for what he could do to help prisoners physically.
He also canonically did not harvest organs. If you consider him doing his job as a surgeon (although he does a little more than he was supposed to) as killing someone then I guess you could consider his body count that way. He also never purposefully failed surgeries to do such a thing.
Shidou persuaded families to give up their families organs when he was on the job, which he did not have to do, which is what he considers "killing a lot of people". Legally, this is not a crime but he should not have tried to persuade them as hard as he did, it was morally wrong. In my interpretation at least, Shidou was in the same situation as the people he tried to persuade after his family got into an accident, but instead of choosing between strangers he had to choose between his kids and wife. Though it wasn't like choosing between all of them as if they were in critical condition, I assume both or one of his kids ended up braindead but his wife was in critical condition and he had to choose to either let her die, or use his childs organs to save her. He ended up choosing to use his childs organs, but his wife still ended up dying. It also seems to show that he is working on his wife, which it is generally unethical to work on your own family because it clouds judgement. It shows in the result of the surgery, his wife dies likely due to a mess up in the surgery because of this.

I feel that generally, westerners have a hard time grasping that this is what Shidou's case is like (For the most part. some of this is purely my interpretation. but he did not do the things you said he did) because taking from brain dead people are less looked down upon than in Japan. It's similar to how divorce is looked at, like in Kazui's case. I understand Shidou's case is one of the more confusing ones but it's sad to see that people still believe he did something completely illegal like that, he is not that morally corrupt
