I don't know about 'accidentally' because it's hard to miss, but the random strumpets in the Witcher 3 constantly make Madonna references - 'it's a material world and I'm a material girl' now I'm mad though because I just thought about the ones you talk to and they're like 'ew, not for all the gold in Skellige!' like wow that's brutal

I found a place in Mario 64 where you can get stuck and have to reset the game if that counts. =P
